Updated Vaccination Information from Putnam County (11 Jan 2021)

The Putnam County Department of Health is preparing to expand COVID-19 vaccination distribution to those aged 75 and older, first responders, teachers and other essential workers who live and/or work in Putnam county.
- Vaccinations are by appointment only. To sign up, start by checking your eligibility.  If you are eligible, you will be given a list of local places where you can get a vaccination.
- Those who are eligible can also call the State COVID-19 Vaccination Hotline for an appointment: 1-833-NYS-4-VAX (1-833-697-4829).
- Prior to receiving the vaccination, you must complete the New York State COVID-19 Vaccine Form; once filled out you will receive a submission ID. Do not show up to a vaccination site without an appointment. You will not be given a vaccine.
